Being told during an abdominal scan that a lesion or mass was found in the liver, with no symptoms leading up to it, is a common and understandably anxious way to first hear about a liver hemangioma. What is a liver hemangioma?
A liver hemangioma is a completely non-cancerous (benign) lump in the liver made of an abnormal cluster of blood vessels. It is not skin or liver cancer, it does not spread to other organs, and it will never turn into cancer.
Liver hemangiomas are classified by size:
Typical (under 5 cm): the most common type. They cause zero symptoms, require no treatment, and are perfectly safe to leave alone.
Giant (5 cm or larger): rare. Because of their size, they can occasionally cause a dull ache in your stomach, clot internally, or carry a very small risk of bleeding.

Symptoms of liver hemangiomas
The vast majority of liver hemangiomas cause no symptoms and are discovered entirely incidentally on abdominal imaging. When symptoms occur they are almost exclusively associated with large or giant hemangiomas:
A dull aching discomfort or heaviness in the upper right abdomen from mass effect on surrounding structures
Early satiety or nausea from compression of adjacent stomach by very large lesions
A palpable abdominal mass in very large lesions
Acute severe abdominal pain from spontaneous haemorrhage, an extremely rare complication in giant lesions
In Kasabach-Merritt syndrome, which is exceptionally rare, bleeding and bruising from platelet consumption within the lesion
For the overwhelming majority of people with a liver hemangioma, the only relevant consequence of the finding is the reassurance that the liver lesion has a benign explanation.
What causes liver hemangiomas?
The exact cause of liver hemangioma development is not fully understood. Contributing factors include:
Congenital vascular malformation liver hemangiomas are thought to arise from a congenital abnormality in the development of hepatic vascular tissue, present from birth or early life even if not identified until adulthood.
Female sex and hormonal factors liver hemangiomas are approximately twice as common in women as in men, and oestrogen is thought to promote their growth. They may enlarge during pregnancy and with oestrogen-containing hormone therapy.
Age hemangiomas become more commonly identified with age, likely reflecting both the increased use of abdominal imaging in older adults and possible slow growth over time.
Genetic factors occasional familial clustering suggests a genetic predisposition in some cases, though most arise without an identified hereditary cause.
How is a liver hemangioma detected?
Liver hemangiomas are detected through abdominal imaging. Blood tests play a very limited role in the assessment of the hemangioma itself but are relevant to overall liver health assessment.

Most liver haemangiomas require no treatment beyond confirmation of the diagnosis and, in some cases, interval imaging to confirm stability. Giant haemangiomas above 5 cm causing significant symptoms or identified in women planning pregnancy with oestrogen-sensitive lesions may be monitored more actively. Surgical resection or other interventional procedures are reserved for the rare cases causing significant symptoms, rapid growth, or complications. Women taking oestrogen-containing hormone therapy should discuss their haemangioma with their doctor, as oestrogen may promote growth. The overwhelming majority of people with a liver haemangioma never require any intervention beyond the initial confident diagnosis.
