What is nasal cavity lymphoma?

Nasal cavity lymphoma is a type of non-Hodgkin lymphoma, a cancer affecting the white blood cells (lymphocytes) within your immune system. While rare, it must be carefully distinguished from common nasal polyps or non-cancerous swelling.

The most common subtypes found in the nasal cavity include:

  • Extranodal NK/T-cell lymphoma (nasal type): a rare, aggressive lymphoma strongly associated with Epstein-Barr virus (EBV). It typically develops in the nasal cavity or surrounding sinuses and can cause local tissue destruction.

  • Diffuse large B-cell lymphoma (DLBCL): an aggressive B-cell lymphoma that can develop in extranodal tissues, including the nasal cavity and sinuses.

  • Marginal zone lymphoma: a slow-growing, low-grade cancer that develops in the moist lining of your nasal passages and sinuses.

  • Other lymphomas: less common B-cell and T-cell lymphoma subtypes can also occur in the sinonasal region.

Symptoms of nasal cavity lymphoma

Nasal cavity lymphoma symptoms are frequently indistinguishable from benign nasal conditions in their early stages, which is why imaging is essential. Common signs include:

  • Persistent unilateral nasal obstruction that does not respond to standard treatments

  • Recurrent or persistent nosebleeds

  • A feeling of pressure, fullness, or pain in the nasal cavity or sinuses

  • Visible or palpable swelling of the nose, cheek, or palate

  • Facial numbness from nerve involvement

  • Crusting and discharge from the nasal cavity

  • Midline facial destruction in advanced extranodal NK/T-cell lymphoma

  • Constitutional symptoms including fever, drenching night sweats, and unexplained weight loss of more than 10% body weight, known as B symptoms, indicating systemic disease

  • Cervical lymph node enlargement

The combination of local nasal symptoms with constitutional B symptoms is a particularly important clinical signal warranting urgent investigation.

What causes nasal cavity lymphoma?

The causes of nasal cavity lymphoma vary according to the lymphoma subtype. In many cases, no single cause can be identified. Factors associated with particular lymphoma types include:

  • Epstein-Barr virus (EBV) is strongly associated with extranodal NK/T-cell lymphoma, nasal type. EBV is detectable in almost all cases of this lymphoma, although most people infected with EBV never develop lymphoma.

  • Impaired immune function HIV infection, organ transplantation, and long-term immunosuppressive treatment are associated with an increased risk.

  • Chronic immune stimulation some marginal zone lymphomas are associated with prolonged immune stimulation caused by autoimmune conditions or chronic infections.

  • Genetic factors certain HLA gene variants are associated with increased risk of developing lymphoma, although these factors vary substantially between lymphoma subtypes.

  • Geographic variation extranodal NK/T-cell lymphoma is significantly more common in Asian and Latin American populations, reflecting both genetic susceptibility and EBV prevalence patterns. not yet fully understood

How is nasal cavity lymphoma detected?

Nasal cavity lymphoma is detected through head and neck imaging combined with blood tests assessing systemic disease markers. A definitive diagnosis always requires tissue biopsy.

Why early detection matters

The outlook for nasal cavity lymphoma varies considerably depending on the lymphoma subtype, stage, and response to treatment. Extranodal NK/T-cell lymphoma, nasal type, is an aggressive lymphoma, but some other lymphomas affecting the sinonasal region can grow more slowly. When lymphoma is suspected, identifying the subtype and determining whether it is confined to the nasal region or has spread elsewhere are important for selecting appropriate treatment.

Because symptoms can overlap with common nasal and sinus conditions, persistent or unusual symptoms may require further investigation. MRI can help identify and characterise a suspicious mass and assess its local extent, while biopsy is required to confirm whether the lesion is lymphoma and determine its subtype.
