What is thrombocytosis?
Platelets are the cells responsible for patching up wounds and stopping bleeding. You need enough of them circulating to respond quickly when needed, but too many can start to cause problems of their own.
A normal platelet count sits between 150 and 400. Once it passes 450, it's called thrombocytosis, and at this point there are enough platelets circulating that clotting risk starts to rise. There are two main reasons this happens:
The body’s reaction (reactive thrombocytosis): The most common cause. The bone marrow itself is healthy, but it's overproducing platelets in response to something else, like an infection, an injury, or low iron. Treat that underlying problem, and the platelet count usually returns to normal.
The source problem (primary thrombocytosis): This is rarer and more serious. It means the bone marrow itself is malfunctioning and producing platelets uncontrollably. This needs specialist care, since the risk of dangerous blood clots or strokes is much higher.
Symptoms of thrombocytosis
Many people with thrombocytosis have no symptoms, especially in reactive cases. When symptoms do occur, most commonly in primary forms, they include:
Neurological: persistent headaches, dizziness, or visual disturbances.
Vascular: burning or tingling in the hands and feet (erythromelalgia).
Clotting risks: increased likelihood of stroke, TIA, or deep vein thrombosis.

What causes thrombocytosis?
Causes depend on whether the condition is reactive (a response to external factors) or primary (a bone marrow issue).
Reactive thrombocytosis:
Iron deficiency anaemia: the most common cause, as the body ramps up platelet production to compensate for low iron.
Infection and inflammation: conditions like arthritis or bowel disease trigger platelet release through inflammatory signals.
Surgery or Injury: counts often peak shortly after major surgery or tissue damage as the body heals.
Spleen removal: losing the spleen removes the primary site where platelets are cleared from the blood.
Malignancy: certain tumors produce cytokines that drive high platelet counts.
Vitamin deficiency: folate and B12 deficiency correction can temporarily elevate platelet count as bone marrow rebounds
Primary thrombocytosis:
Essential thrombocythaemia (ET): a clonal myeloproliferative neoplasm; frequently associated with JAK2 V617F mutation (present in ~50–60% of ET cases) or CALR/MPL mutations
Polycythaemia vera: elevated red cells with associated thrombocytosis
Myelofibrosis: bone marrow scarring with abnormal platelet production
How is thrombocytosis detected?
