What is splenomegaly?
Splenomegaly is an enlarged spleen, typically defined in adults as a spleen measuring more than about 12–13 cm in length on imaging. Located in the upper left abdomen, the spleen filters damaged blood cells, stores platelets, and fights off infections.
Doctors classify an enlarged spleen by the underlying mechanism driving it:
Congestive: caused by high blood pressure in the liver (portal hypertension) or veins, which forces blood to back up and pool inside the spleen like a clogged sponge.
Infiltrative: caused by a buildup of abnormal cells or materials, such as cancer cells in leukemia and lymphoma, or unusual proteins in rare metabolic storage disorders.
Immune and Inflammatory: caused by the spleen overworking its immune defenses, which triggers tissue swelling during viral infections (like glandular fever) or chronic autoimmune diseases.
Haematological: caused by the spleen working overtime to destroy massive amounts of damaged red blood cells, or trying to generate new blood cells because the bone marrow is failing.
Symptoms of splenomegaly
Many people with splenomegaly have no symptoms and only discover it during abdominal imaging performed for another reason. When symptoms do occur, they may reflect the enlarged spleen itself or the condition causing it:
Left upper quadrant discomfort, fullness, or a dull aching pain, often worse after eating
Early satiety from the enlarged spleen compressing the stomach
Referred pain to the left shoulder from diaphragmatic irritation
Fatigue and weakness from anaemia caused by increased red cell destruction or bone marrow suppression
Increased susceptibility to infection from splenic dysfunction or underlying immune impairment
Easy bruising and bleeding from thrombocytopenia, as an enlarged spleen sequesters platelets excessively

Splenic rupture, though uncommon, is the most serious acute complication of splenomegaly and can be spontaneous in massive enlargement. Sudden severe left upper quadrant pain with haemodynamic instability is a medical emergency.
What causes splenomegaly?
Splenomegaly results from diverse underlying conditions affecting the spleen through different mechanisms. Common contributing causes include:
Liver disease: cirrhosis and high portal blood pressure force blood backward into the spleen, causing severe congestion and swelling.
Blood cancers: lymphoma and leukemia (like CML or myelofibrosis) flood the spleen with malignant cells, often causing massive enlargement.
Viral infections: conditions like Epstein-Barr virus (glandular fever), HIV, and hepatitis trigger a temporary, immune-driven swelling.
Autoimmune conditions: diseases like lupus, rheumatoid arthritis, or hemolytic anemia cause chronic inflammation or force the spleen to destroy too many red blood cells.
Heart failure: right-sided heart failure causes venous blood to back up into abdominal organs, congesting the spleen.
Genetic and metabolic disorders: conditions like sickle cell anemia, thalassemia, or Gaucher's disease overwork the spleen or cause cellular debris to accumulate inside it.
Tropical infections: globally, widespread parasitic infections like malaria are leading drivers of extreme spleen enlargement.

Treatment targets the underlying cause rather than the spleen itself. Portal hypertension from cirrhosis is managed with beta-blockers, endoscopic variceal treatment, and TIPS procedure in refractory cases.
